FMCA Portfolio Guidance and Template

Please ensure you have read the following two documents before downloading the template:

• FMCA Portfolio Guidance

Use the FMC Portfolio template to complete your portfolio for accreditation. There are different templates for each section of the portfolio.

Click on the title of the documents below to download a word version of the template.

The portfolio should be submitted as four separate PDF documents as follows.

1. PDF DOC 1 of 4: First Stage Documentation
2. PDF DOC 2 of 4: Primary commentary (s) and evidence of practice. Choose either
   • PDF2A: Primary Evidence AIM Case; or
   • PDF2B: Primary Evidence (No AIM Case submitted)
3. PDF DOC 3 of 4: Secondary commentaries: one Child and one Financial
4. PDF DOC 4 of 4: PPC Feedback and Reflective Account
